Geldt voor lactulose: oraal poeder en oplossing

Bijwerkingen zijn onder meer:

Gasvormige distentie, oprispingen, winderigheid, borborygmi, abdominaal ongemak (bijvoorbeeld kramp). Uitdroging en hyponatriëmie bij zuigelingen.

Toepassing op lactulose: samengestelde oplossing, oraal poeder voor reconstitutie, orale oplossing, orale siroop, orale en rectale vloeistof

Gastro-intestinaal

Diarree is een indicatie van overdosering. Ernstige diarree kan leiden tot hypovolemie, hypokaliëmie en hypernatriëmie, vooral bij oudere of acuut zieke patiënten. Als diarree optreedt, moet de dosering worden verlaagd.

Pneumatosis cystoides intestinalis kan optreden bij lactulosetherapie als gevolg van verhoogde intracolonische druk veroorzaakt door ammoniaksekwestratie in de darm, en met name als er tegelijkertijd sprake is van een inbreuk op de integriteit van het darmslijmvlies.

Gastro-intestinale bijwerkingen zijn onder meer buikkrampen, gasvorming, winderigheid, oprispingen, darmstuwing, kramp, misselijkheid en braken, en bij excessieve doses diarree. Colonverwijding is gemeld bij oudere patiënten. Zeldzame gevallen van pneumatosis cystoides intestinalis zijn gemeld.

Metabole

Metabole bijwerkingen zijn onder meer hypernatriëmie, hypokaliëmie en hyperchloremische metabole acidose.

Vocht- en elektrolytstoornissen, waaronder ernstige hypokaliëmie, hypernatriëmie en hyperchloremische metabole acidose, treden gewoonlijk op als gevolg van ernstige diarree en daaropvolgend vochtverlies. Er is ook een geval gemeld van fatale lactulose-geïnduceerde melkzuurocidose in afwezigheid van diarree. Oudere en/of acuut zieke patiënten kunnen een verhoogd risico lopen op nadelige metabolische effecten met lactulose.
